Service FAQs

Heart of Gold Physical Therapy is an out of network provider.  A detailed invoice is available upon request for insurance reimbursement and services qualify for HSA/FSA benefits.  Payment for services rendered is due at the time of your appointment.  

One third of the practice is reserved for reduced fee patients and is based on a sliding scale.

Therapy Services:

• NEW Lymphedema Screening and Prevention Program 

• Discovery Call - 15 min

• Initial Consultation, 75 min

- In-depth evaluation and treatment, plan of care development

- Prevention screening and compression fitting included

• In Person Physical Therapy, 60 min

• Compression Garment Fitting, 45 min 

Package rates and gift cards are available.

Reduced rates offered for a series of lymphedema treatment visits.

Accepted forms of payment: Check, cash, credit card.

Direct Access:

It is not required to have a physician referral to begin treatment.  We will ask for a referral following 45 days of treatment or your first 12 visits.  Wellness visits do not require a referral.
